WAVES Families,

Thank you for registering to join the Chatsworth WAVES this summer.  We're excited for another fun season of summer swim.  Now all we need is some warm weather!

We have your swimmer(s) registered and are beginning to assemble the roster for county submission and also finalize our practice schedule. Please complete your registration with payment NO LATER than April 25th.  Our team is quite full with 125 swimmers and registration will close soon.  We need your payment to secure your spot. If you are receiving this email in error and have already paid, please let us know ASAP so we can track it down.

You may pay online or by check.  Checks are to be made payable to CHATSWORTH WAVES and may be mailed/dropped of at my house in Chatsworth:

We also require 3 completed forms located in the LINKS section & via this email of the site.  They are:

• Vacation & Volunteer Form
• Medical Release
• Concussion Form (you may put all swimmers names on this one form)
